To be considered for placement at New Dawn you will have to have the following things completed and faxed to our Administration office.

• A Drug and Alcohol Evaluation completed within the past 90 days. (If you don’t have an evaluation scheduled or completed already you can call ABC at 605-347-2991 to schedule ** We do not accept collect calls**)

• A physical completed within the past 90 days. (ABC does not provide physicals but you can call a local provider)

• A TB test within the past year.

• A Pre-intake form (Click link above to view).

All of the above documents can be faxed to 605-456-9892. Once all documents have been received our clinical team will review the file for placement.
